Annual Revalidation (certificate-holders only)

BEAL Testing Services (BTS) carries out an annual review and revalidation of your quality documents and processes in order to update the Appraisal certificate and arrange an audit at your office and building site/s. The first step is to complete this form and pay the revalidation fee invoice at least 3 weeks before the expiry date. This is to allow enough time for BTS to process the Appraisal revalidation and issue an updated certificate before the expiry date. Late return of the revalidation form and or payment of the invoice may result in the Appraisal being suspended.

Notes a) You can advise BTS of any new updates to your BPQP, or Technical & Installation Manual, or drawings, or components when you complete this Revalidation Form. During the revalidation process BTS will evaluate the changes for inclusion on the renewed certificate; b) There may be a fee to cover the unanticipated work required to evaluate and approve the changes. You will be informed and asked to approve additional costs before evaluation work proceeds; c) All changes are subject to approval being given in writing by BTS prior to any public release; d) Any changes to any Appraisal documentation without prior written approval of BTS will be deemed a critical non-conformance of your BPQP and the Appraisal will be suspended within 48 hours, with written notice to the Appraisal Holder.

Section 2 – Preparation for the Annual Surveillance Audit
Notes: a) The annual audit is carried out at your office (for the BPQP) and a building site or sites to observe a trained and approved applicator or installer using the checklists; b) The surveillance audit will be organised by an Auditor who will contact you to arrange a suitable date and time.
